Incidents or concerns that involve sexual assault, sexual misconduct, intimate relationship violence, or sexual harassment.
Bias, Discrimination, and/or Harassment
Incidents or concerns that you believe to be based on actual or perceived race, ethnicity, national origin, citizenship status, religion, sex, sexual orientation, gender identity and expression, physical or mental disability, age, veteran or military status, marital status, or other legally protected.
Concern About the Well-being of a Student
Concerns regarding the physical, emotional, or mental well-being of a student. The concerns might include things like anxiety, depression, eating disorder, drug or alcohol consumption, bizarre or uncharacteristic behavior/thoughts, or thoughts about suicide/harm to others.
Disruptive or Aggressive Behavior
Behaviors or incidents that interfere significantly with normal living and working conditions, or that involve vandalism, graffiti, damage or destruction of personal or university property.
Hazing and/or Organizational Misconduct
Actions or situations that recklessly or intentionally endanger mental or physical health, or create substantial embarrassment, harassment or ridicule, or involve the forced consumption of food, alcohol or drugs in the course of initiation or continuing affiliation with an organization.
