Medical Requirements for New Students

Requirements For All New Students

Colgate University requires all new students to complete and submit health information before the start of class. This information is necessary to comply with University and New York state requirements to help us direct your health care during your time at Colgate.

The deadline for submission is July 1 for the fall semester and January 9 for the spring semester.

Your Checklist:

  • Activate your Patient Portal account (you should receive a separate email with instructions). You will access and submit all required medical information through your confidential medical portal.
  • Complete the Insurance Wavier

Required Forms

Colgate University Student Health Services reviews your medical information and verifies immunization requirements through the confidential medical portal. 

Colgate University requires the completion of the following:

  • Medical History 
  • Required immunizations and uploaded official immunization records, copies of blue cards, yellow cards, or state immunization records.
    • (Required: Two doses of MMR, one dose of Meningitis ACWY within the last 5 years, Two doses of Varicella, and a Tdap within ten years).
  • NYS Meningitis Response form if refusing Meningitis ACWY
  • TB Screening Questionnaire
    • Submission of TB testing (if required) based on your answers to the TB Screening Questionnaire

      You will find these requirements and instructions on submitting the required health information on the colgate.edu/new-student checklist. You will receive a Colgate email in late June with instructions on activating your Patient Portal account with Student Health Services.


Students who lack adequate immunizations and/or do not submit immunization information will be unable to check in and receive their room key upon arrival. We strongly recommend you obtain all required vaccines before you arrive at Colgate. If that is not possible, you can receive your immunizations (at cost) through Student Health Services after your arrival on campus.

Medical or Religious Exemptions

If you are applying for a medical or religious exemption for any immunizations, submit your completed forms online via myColgateHealth Patient Portal or email your completed form to studenthealth@colgate.edu.

 

Submit Additional Forms

Some students need to share additional medical information with Student Health Services for continuity of care while at Colgate University, such as:

  • Allergy injection records and orders
  • Medication Infusions (Remicade, etc.)
  • Medication injection records and orders (Humira, Somponi, Dupixent, Stelara, Tremfya, Depo-Provera, Xolair)
  • Documentation of any medical conditions for which you will need Student Health Services to provide prescriptions or ongoing care

Submit your forms online using the myColgateHealth Patient Portal or email your forms to: studenthealth@colgate.edu.

Additional Information

As you prepare for your arrival at Colgate, we recommend putting together a first-aid kit to have on hand in case of minor illness and injuries.

  • Hand sanitizers
  • Pain/fever reliever (acetaminophen, ibuprofen)
  • Prescription medications and lockbox

 

Consider Bringing

  • Antacids/Anti-gas Tablets or Liquid
  • Antibiotic cream
  • Anti-itch cream
  • Antihistamine (non-drowsy)
  • Band-aids
  • Cold/hot pack
  • Sore throat medicines
  • Tissues

Wilderness Adventure requires the submission of a separate health history form, distinct from the forms above that is submitted to student health services.
