Insurance Forms

Full-time students must either enroll in (and be billed for) the Colgate Student Health Insurance Plan, or waive enrollment by providing proof of comparable coverage.

For the 2014-2015 academic year, please visit www.gallagherstudent.com/colgate to waive or enroll and to find more information on this year's plan. The deadline for waiver/enrollment submission for 2014-15 wass Septmeber 8, 2014.

Students wishing to make changes in their insurance coverage during the academic year should contact the student accounts office at 315-228-7812.

Plan features:
  • An unlimited policy year maximum
  • Low annual deductibles: $100 single, $200 family in-network and $200 single, $400 family out-of- network
  • Coverage at 90% in-network & 70% out-of-network
  • In-network physician’s office visits covered at 100% after a $15 copay
  • Preventive care services with no deductibles, copays or coinsurance (In-Network Only)
  • Students will be able to locate providers whether at home, school, or throughout the country through the CDPHP Network
  • Prescription drug coverage with low copays
  • Coordination with the on-campus Student Health Center
  • Pediatric dental and vision coverage up to age 19
  • Worldwide Coverage including medical evacuation, repatriation & travel assistance services
  • Wellness discounts including vision and dental

Waiving Enrollment

Students wishing to waive enrollment in the Colgate Student Health Insurance plan should complete the online waiver form each summer, providing information about the existing coverage, which Colgate will record as part of your health record.

Markel Insurance Refunds
