Students who are applying for graduate fellowships, employment, or post- graduate work in law, the health professions, or other graduate education, should know that a disciplinary record may need to be reported if such information is requested. Such information may also be required for other applications for which a student is required to complete. Typically this information is asked directly of the student, but some opportunities also require a certification of a student's suitability to be completed by a university official, which is typically the student's administrative adviser.
